Documents you'll complete online
Once approved, you upload everything through your secure owner-operator portal. Our team reviews each document and tells you immediately if something needs to be replaced.
Completed Carrier Agreement / Contract
Signed carrier agreement returned in full.
- Complete the contract
- Initial every page
- Return all 10 pages
- Sign twice on page 6
- Write your name on page 3
- If registering under an EIN, include the company name
- Use a personal email address rather than a company email where required by the registration instructions
Vehicle & Driver Information Sheet
Completed vehicle and driver information sheet.
- Complete every field
- Include accurate vehicle details
Completed W-9
Current IRS Form W-9.
- Use the legal name matching your SSN or EIN
- Sign and date the form
Vehicle Registration
Current registration for the vehicle you will operate.
- Must be current and legible
Driver's License
Valid driver's license, or CDL where applicable.
- Front of the license
- Must be valid and unexpired
Social Security Card OR EIN Letter
Social Security card, or IRS EIN letter if operating under an EIN.
- Submit one of the two documents
Voided Check for Direct Deposit
Voided check or bank letter for direct deposit setup.
- Write VOID across the check
- Account and routing numbers must be legible
3-Year Motor Vehicle Record (MVR)
Motor vehicle record covering the previous three years.
- Must cover the previous 3 years
- Must be issued within the last 3 months
ACORD Insurance Certificate
Certificate of insurance meeting the coverage requirements.
- Minimum $1,000,000 combined single liability limit
- Minimum $100,000 cargo liability
- Collision coverage recommended but not mandatory
- Scheduled drivers listed
- Scheduled vehicles listed
- TRANZ AMERICA LLC shown as Additional Insured
- TRANZ AMERICA LLC shown as Certificate Holder
Vehicle Photos
Photos of the vehicle from every required angle plus measurements.
- Front, rear, left side, right side
- License plate
- Cargo space
- Vehicle length, wheelbase and height measurements
